receiving and taking the answer, plea, demurrer, disclaimer, allegation or examination of any party or parties to any action

the examination of any witnesses upon any interrogatories or de bene esse or in chief or on any other occasion

swearing executors and administrators

any persons in any action, matter or proceeding which is pending or about to be instituted in any court
any other matters as may be provided by any laws in Malaysia
HOW TO GET THE SERVICES

You may come to our business premises at our prescribed business hours. For your convenient, it is advisable to request for an appointment with our Commissioner for Oaths or enquiring his availability by calling our telephone number as stated herein.

The deponent who is making the declaration, affirmation, affidavit, oath or statement is required to APPEAR IN PERSON.

Please ensure that the document is not signed prior to your attendance before our Commissioner for Oaths and that all the relevant particulars (Name, IC/Passport No., address, etc.) in the document are in order or written or completed, where necessary. The date of the document shall be the date of your signing of the document before our Commissioner for Oaths.

Please bring your identification proof (original) for verification purpose. For Malaysian : IDENTITY CARD and for Non-Malaysian : PASSPORT.

Our Commissioner for Oaths will personally attend to you :

Verifying your identity, personal particulars and address.

Read over and explain the contents of the document and exhibit attached thereto (if any). Jurat will be used in the event that the deponent is blind, illiterate or does not understand the language written in the document.

Initial any alteration made in the document.

You are required to sign or affix your thumb print on the document and the Register Book of our Commissioner for Oaths in the presence of our Commissioner for Oaths.

Our Commissioner for Oaths will affix his seal to the document.

Kindly pay the prescribed fees to our Commissioner for Oaths. Please request receipt for such payment.

Our Commissioner for Oaths may require the document to be photocopied for his record, where necessary.

Before leaving our premises, please ensure you have obtained the document, receipt for payment as well as your IC or passport.
Kindly take note that our Commissioner for Oaths shall refuse to provide the services in the following events:-
(i) the signatory to the document does not appear in person or the document is not signed or executed before our Commissioner for Oaths.
(ii) our Commissioner for Oaths has cause to suspect that any person before him is engaging in deception, fraud, duress or any other illegal conduct.
(iii) request for services other than at place of business stated herein except with approval of the Court.
(iv) certifying true copy of the document is not allowed.
(v) the document is not authorised by any laws in Malaysia to be executed before a Commissioner for Oaths.
FEES
Pursuant to the provisions under Section 4 of the Statutory Declarations Act 1960 read with
the provisions under the Statutory Declarations (Fees) (Amendment) Order 1993 :

1 original Statutory Declaration : RM4.00
1 duplicate Statutory Declaration : RM2.00

Pursuant to the provisions under Rule 17 of the Commissioners for Oaths Rules 2018,
the fees to be levied by a Commissioner for Oaths shall be as in the Second Schedule :-

1 original Affidavit : RM10.00
1 Exhibit in original Affidavit : RM5.00
​1 duplicate Affidavit : RM5.00
1 duplicate Exhibit : RM2.00
1 Document, Affirmation, Acknowledgement, Deed or any other Instrument : RM10.00
1 duplicate of Document, Affirmation, Acknowledgement, Deed or any other Instrument : RM5.00
